Certificate of Completion

The Center for Organizational Excellence at Washburn University recognizes individuals with a Certificate of Completion in Lean Six Sigma: Green Belt and Black Belt.

Green Belt Requirements

Participation 

  • Complete all class and homework assignments.
  • 100% attendance required. Class content must be made up if missed.

Exam

  • Minimum average exam score of 90%

Project

  • Must be completed as defined in project completion criteria. 
  • Final presentation of completed project is required.

Once the program consultant advises that an individual has completed the requirements, the Center on Organizational Excellence will notify the individual by letter recognizing the consultant report.

The individual will be directed to submit a brief project summary report in order to meet full eligibility for award. The project summary report must be received by July 1 or December 1 in order to be eligible for the subsequent awards event. 

Black Belt Requirements

The Center for Organizational Excellence at Washburn University recognizes individuals with a Certificate of Completion in Lean Six Sigma: Black Belt. 

Participation

Project

  • Must be completed as defined in project completion criteria.
  • Final presentation of completed project is required.

Semi-Annual Awards

The Center for Organizational Excellence awards the Professional Certificate of Completion twice a year – generally in August and January. Individuals who have completed their project and have conducted a final report out per the approval of the Lean Six Sigma consultant should submit a brief summary of the LSS project for inclusion in a program book that is distributed at the event semi-annual event. The summary should only be one to two pages sent as a WORD document attachment following this structure:

  • Title
  • Author
  • Problem
  • Approach
  • Results

Since a mixed audience will likely be viewing the program book, proprietary information should not be included. The summary must be received no later than July 1 in order to receive the award in August; December 1 in order to receive the award in January.

An invitation to the award recipient and a guest will be sent approximately one month in advance.
