Employee Wellness Program Tiers

Tier 1: Completion of an Annual Health Assessment

Employee Wellness Program Online Enrollment (Open August 20-Sept 5)

The annual health assessment is offered at no cost to the employee and is all that is required to receive a tier 1 incentive. The health assessment includes two parts. The first part is completing a health screening. The health screening consists of a finger stick to gather a small sample of blood to test for cholesterol, triglycerides, and glucose. Additional tests will include weight, blood pressure, body fat percentage, BMI, and waist circumference. The testing is performed by a third party, Health Works, Inc. (Health information obtained in this health screen is confidential and personally identifiable information will not be released to any person without written consent.  Aggregate data will be compiled for statistical purposes and will not include participants personally identifiable information.  Participants may or may not choose to release personal health information for use only by the Washburn Wellness Staff to evaluate health status, needs or progress.) The second part of the health assessment is completing the Personal Wellness Profile (health risk assessment). The goal of the health assessment is to provide individuals with an increased awareness regarding their health and is not meant to take the place of an annual physical exam. The Personal Wellness Profile is designed to provide a complete health and lifestyle assessment based on the most current scientific guidelines for optimal health. (The health risk assessment is provided by Wellsource Inc. For access to their privacy statement click on the following link: Wellsource Privacy Statement)
