
The annual health assessment is offered at no cost to the employee and is all that is required to receive a tier 1 incentive. The health assessment includes completing a: 1) health screening and 2) Personal Wellness Profile (health risk assessment). The goal of the health assessment is to provide individuals with an increased awareness regarding their health and is not meant to take the place of an annual physical exam.
The Personal Wellness Profile is designed to provide a complete health and lifestyle assessment based on the most current scientific guidelines for optimal health. The health screening includes a blood lipid profile, blood pressure check, and body composition measurement.
Employees completing tier 1 can choose to get started on the path to a healthier lifestyle by completing activities and/or behaviors listed on the Tier 2 Wellness Action Sheet.

Employees demonstrating the behaviors listed on the Tier 2 and Tier 3 Wellness Action Sheets will be eligible for tier 2 and tier 3 incentives respectively. Action sheets are available to all employees officially enrolled in the Employee Wellness Program via the Employee Wellness Coordinator. Progress is self-reported and documented by participating employees through submission of wellness action sheets to the Employee Wellness Coordinator.
