Employee Wellness Program Tiers
Tier 1: Completion of an Annual Health Assessment
The annual health assessment is offered at no cost to the employee and is all that is required to receive a tier 1 incentive. The health assessment includes completing a: 1) health screening and 2) Personal Wellness Profile (health risk assessment). The goal of the health assessment is to provide individuals with an increased awareness regarding their health and is not meant to take the place of an annual physical exam.
The Personal Wellness Profile is designed to provide a complete health and lifestyle assessment based on the most current scientific guidelines for optimal health. The health screening includes a blood lipid profile, blood pressure check, and body composition measurement.
Tier 2: Healthy Behaviors: Getting Started
Employees completing tier 1 can choose to get started on the path to a healthier lifestyle by completing activities and/or behaviors listed on the Tier 2 Wellness Action Sheet.
Examples of Tier 2 Activities and Behaviors
- Meet with the Employee Wellness Coordinator to discuss your Personal Wellness Profile results.
- Have a current membership at a fitness facility.
- Read a suggested article about how to avoid job burn out.
- Wear a seat belt 100% of the time

Tier 3: Healthy Lifestyles: Getting It Done
Employees completing tiers 1 and 2, and living a healthy lifestyle can be rewarded by submitting a Tier 3 Wellness Action Sheet.
Examples of Tier 3 Activities and Behaviors
- Complete a preventive exam with a health care provider.
- Exercise moderately for at least 30 minutes most days of the week
- Refrain from driving while using a cell phone.
- Acquire certification in CPR and/or First Aid

Employees demonstrating the behaviors listed on the Tier 2 and Tier 3 Wellness Action Sheets will be eligible for tier 2 and tier 3 incentives respectively. Action sheets are available to all employees officially enrolled in the Employee Wellness Program via the Employee Wellness Coordinator.
Progress is self-reported and documented by participating employees through submission of wellness action sheets to the Employee Wellness Coordinator.
Employee Wellness Program Incentives
Tier 1 Incentives
Employees completing Tier 1 can choose to receive one of the following participation incentives:
- $75 Cafeteria Plan (Flex Spending Account) reimbursement*
- $75 fitness center membership reimbursement**
- $75 worth of Bod Bucks placed on the participating employee's iCard***
Tier 2 Incentives
Employees completing Tier 2 can choose to receive one of the following participation incentives:
- $25 Cafeteria Plan (Flex Spending Account) reimbursement*
- $25 fitness center membership reimbursement*
- $25 worth of Bod Bucks placed on the participating employee's iCard***
Tier 3 Incentives
Employees completing Tier 3 can choose to receive one of the following participation incentives:
- $50 Cafeteria Plan (Flex Spending Account) reimbursement*
- $50 fitness center membership reimbursement*
- $50 worth of Bod Bucks placed on the participating employee's iCard***

**Employees selecting this incentive must have an existing membership to the SRWC or other health club in order for the incentive to be applied to their paycheck.
***All employees eligible to participate in the EWP should already possess a Washburn identification card. The incentive transaction will appear on the employee's paycheck, and Bod Bucks will be placed on the employee's iCard. Bod Bucks can be spent at the University Bookstore, Union Cafeteria, Union Corner Store, and any vending machine accepting iCards.
