DMS Application

This application is to determine if you meet the minimum qualifications of the sonography program. If it is determined that you do, we would then move on to clinical placement procedures. Full acceptance into the program is contingent on a signed affiliation agreement.

Incomplete applications will be scored accordingly or discarded. There are some specific instructions that you must follow for us to evaluate your application.

Instructions:

  • Please watch the instructional video and read this online form thoroughly before applying. An updated instruction video will be posted prior to the new application period.
  • The sonography application should be completed AFTER your 4 hour observation has been completed.

To fill out this application you will need your:

  • Clinical placement information (if you have one already)
  • Resume
  • Official Transcripts
  • Email addresses of two people who can provide letter of recommendations
  • Email address of the sonographer you observed
